At Total Denture Care, accountability and respect are two of our key values so we will do our best to protect the privacy of our valued patients and others that interact with our clinic. Our Privacy Policy demonstrates how we collect, store, use and disclose your personal information, and how we manage the quality and security of your personal information.

We stand committed to do everything in our control and understanding to maintain privacy and comply with the Privacy Act 1988 and other relevant laws and regulations.

 

What is Personal Information?

‘Personal Information’ refers to any information or opinion, whether true or not, and whether recorded in a material form or not, about an identified individual or an individual who is reasonably identifiable. In general terms, this includes information or an opinion that personally identifies you either directly (e.g. your name) or indirectly.

This may include, but is not limited to:

  • Date of Birth
  • Telephone Number
  • Email address
  • Emergency Contact
  • Private Health Insurance Details
  • Department of Veterans Affairs (DVA) Details
  • Dentist/ other health practitioner’s name and contact number
  • Medications
  • Copies of any referrals and any dental reports relating to the patient
  • Any other details provided directly by the patient

Should we collect your personal information, we will do so only with your consent or in the instance you have provided consent to another health professional to do so, if it is necessary to prevent a serious and imminent threat to life or health, or as otherwise required or authorised by law, and we take appropriate measures to protect the security of this information.

 

How do we collect your Personal Information?

We collect your personal information directly when you or a family member/ carer/ other relevant person:

  • interact with us over the phone;
  • interact with us in person;
  • interact with us online;
  • are referred by another health professional to our clinic

 

Collecting and Disclosing Personal Information from Third Parties

Total Denture Care may also collect your personal information from third parties or through publicly available sources.

Total Denture Care will disclose personal information to a third party where it is necessary for the provision of the service and the management of the clinic.

Where personal information is disclosed to third parties, Total Denture Care seeks to ensure that they comply with the requirements of the Privacy Act. However, Total Denture Care is not responsible for the misuse of personal information by these third parties.

While Total Denture Care takes all reasonable steps to ensure that these parties have and enforce an adequate privacy policy, it is not responsible for the misuse of personal information by these third parties.

 

How do we use your Personal Information?

We use personal information for many purposes in connection with our services, including the following purposes:

  • providing reminders on regular check-ups;
  • providing information on our services;
  • delivering a more personalised experience and service offering;
  • as required for the delivery of the health service to the patient including the manufacturing of the dental prosthetic;
  • improving the quality of the services we offer;
  • as required for the effective operation of our services such as referring a patient to a dentist or dental specialist;
  • for quality assurance, training, billing, debt recovery and as may be required by Total Denture Care’s insurers or industry body;
  • liaising with government offices such as DVA or The Department of Health or Health Fund Insurance Providers, regarding benefits and for claims processing;
  • liaising with other health professionals to enhance treatment outcomes;
  • measuring patient satisfaction;
  • internal administrative purposes;
  • marketing and research purposes;
  • as required under compulsion of law;
  • where there is a serious and imminent threat to an individual’s life, health, or safety; or a serious threat to public health or public safety

 

How do we protect your Personal Information?

At Total Denture Care we will take reasonable steps to ensure that the personal information that we hold is kept confidential and secure by:

  • ensuring technological measures are in place (such as anti-virus software);
  • using only reputable software programs that have high protection and privacy measures;
  • ensuring staff adhere to record keeping guidelines;
  • enhancing physical security of our premises;
  • taking measures to restrict access to only relevant personnel who need personal information to provide our service

 

Online Activity

Whilst we use reasonable measures to ensure that the collection of personal information is done so in a secure environment (our website protects data with an SSL encryption and secure form submissions) no data transmission over the internet can be guaranteed as totally secure. Total Denture Care does not accept responsibility for the security of information transmitted over the internet.

Our website www.totaldenturecare.com.au uses cookies. A cookie is a small file of letters and numbers the website puts on your device if you allow it. These cookies recognise when your device has visited our website(s) before, so we can distinguish you from other users of the website.

We do not use cookies to identify you, just to improve your experience on our website. We may send direct marketing communications and information about yearly check-ups, our services, opportunities, or events that we consider may be of interest to you. Holding of Personal Information

We will not keep personal information for longer than we need to. In most cases, this means that we will only retain your personal information for the duration of your relationship with us unless we are required to retain your personal information to comply with applicable laws, for example dental record-keeping obligations.

How to Access and Correct your Personal Information

At Total Denture Care, we will do our very best to keep personal information accurate and updated.

In the event that you wish to access personal information, this will need to:

  • Be in writing, and;
  • State the full name, date of birth and address of the person, and;
  • Identify the dental information they wish to access and in which form;
  • Where the request is to amend, the information the person claims is necessary to complete or update the dental information.
  • For security reasons, appropriate photo identification may be required prior to access being granted.

Alternatively, should you wish to access information over the phone, security questions may be asked to identify the patient.

Patient information will be shared within Total Denture Care for administrative and billing purposes, also with any other consulting dentist or dental specialists.

Total Denture Care may access information:

  • Provided directly by the patient;
  • Provided on the patient’s behalf with the patient’s consent;
  • From a dental/ health service provider who refers the patient to Total Denture Care or from dental/ health service providers to whom patients are referred.

Total Denture Care takes reasonable steps to ensure that the personal information it collects, uses or discloses is accurate, complete and current. However, the accuracy of the information held depends to a large extent on the information provided by the patient. If you are aware that any of your personal information has changed, please advise us of the change.

Complaints and Queries

For complaints about how Total Denture Care handles, processes or manages your personal information, please contact us. Note we may require proof of your identity and full details of your request before we can process your complaint.

Please allow up to seven business days for Total Denture Care to respond to your complaint. We will do our very best to come to an amicable solution.
